Clubs can now nominate themselves or another club using the nomination forms relevant to the type of club they are and the category they fall into.
The FA Community awards works in partnership with McDonalds to recognise and reward the hard work that goes on around the country week in, week out, to provide the best possible experience for those participating in grassroots football.
The nominations are then judged by an independent panel and winners are selected and annouced at the Hertfordshire FA County Awards held in June every year.
